if you are already married and you are the us citizen, than it would be spouse visa, and done through the embassy. if, then fiancee, and you have to get married withing 90 days from the entry. i guess fiancee is better since you find to be together while the residency is being processed, while you will have to wait longer for them to process it contained by embassy...
You can't get it within a short period of time. You have to be patient once you start.
You own to decide where you want to marry and if you want to live together immediately after your matrimonial or be separated for a while.
