Answers:
japan does not allow dual citizenship.

if you have dual citizenship (as you claim) then you will be required by Japan, to MAKE a choice as to which one you will keep hold of, when you are 18 years old.

Your japanese mom can become a UK citizen, but she WILL LOSE the japanese citizenship in the process.
If she is claiming UK citizenship through marriage to your dad, she would know whether she has it or not. To gain citizenship through marriage to a UK citizen, you obligation to live in the country for 3 full years, and then file a naturalization application. It is not automatic, resembling being born to a parent of a given citizenship.
If she had gained residence through the matrimonial, and never applied for citizenship, she gave up/lost her residence upon divorce and her return to her own country. Can't tell exactly without more details, but her visa is expired and she no longer have any eligibility for UK citizenship.
