c. Degree program and field of study.

d. Date of the termination of enrollment and the reason for termination.
If you already stopped studing at some point, then you are out of status from that point. Your school is required to report to USCIS when you dropped out. If you remain out of status for over 6 months, you are ineligible for any sort of visa for at least 3 years.
